Responsible as a member of the IDG team to the patient care coordinator, attending physician, and/or hospice medical providers for the provision of a comprehensive skilled nursing assessment and planning of care in order to maximize the comfort and health of patients, families, and caregivers consistent with UHS policies and procedures.
