MRH WELCOMES NEW TRANSITIONS PROGRAM COORDINATOR

Marlette Regional Hospital Welcomes New Transitions Program Coordinator ~ Marlette Regional Hospital (MRH) is pleased to welcome our new Transitions Coordinator, Mindy VanBergen, LPN. Mindy joins MRH with years of experience as an LPN in the hospice field and is excited to put her stamp on our program.

Our Transitions program aims to provide support to individuals facing a life-limiting illness and their families. Mindy will meet with patients and families in their homes and assess needs for community resources, provide information on how to connect to these resources, and help them navigate the area offerings.

The Transitions program utilizes specially-trained volunteers who donate their time to offer companionship and support to individuals who may be receiving remedial care or are not ready for hospice services. Volunteers provide practical assistance including helping with errands, light housekeeping, meal preparation, transportation, and respite for family members or caregivers. Transitions volunteers provide friendship and support to help individuals and families cope with the challenges of illness, medical treatments, and outcomes.

Transitions services are available at no charge to anyone with a life-limiting illness. Individuals living at home, with family, in a skilled nursing facility, or an assisted living facility may receive our services.
