Marlette Regional Hospital Family Practices Earn PCMH Designation from Blue Cross Blue Shield of Michigan ~ Marlette Regional Hospital is pleased to share that Blue Cross Blue Shield of Michigan has designated Family HealthCare of Brown City, Kingston Family HealthCare, Marlette Family HealthCare, and North Branch Family HealthCare as Patient-Centered Medical Home (PCMH) practices. The designation period runs from September 1, 2020 through August 31, 2021.
With the PCMH model of care, our family practices establish medical teams that work in partnership with our patients to keep them healthy and monitor their care on an ongoing basis. This is accomplished through technology like our electronic medical record system that helps our care team track and monitor patients’ conditions and offer up-to-date recommendations for preventive care and chronic disease management. With an enhanced focus on educating our patients and keeping them informed, patients learn to better manage chronic conditions like asthma and diabetes.

Housed in the family healthcare offices, our Care Coordinator RNs work one-on-one with patients to educate them about disease processes and self-management at home.
