POSITION SUMMARY:      Position requires a broad knowledge of clinical care and systems management, care coordination expertise, prudent nursing judgment, health care re-imbursement, sound problem solving skills, independent thinking, excellent organizational and interpersonal skills, creativity, flexibility and the ability to multi-task.

Works closely with the Health Care team to identify high-risk complex patients, coordinating and evaluating population based disease management.   The Care Coordinator is responsible for coordinating, monitoring and measuring clinical outcomes of complex patients.  Goals are to achieve positive clinical outcomes, increase the use of the patient registry and electronic health record and lower health care costs.  Assists in the management of patients with complex conditions, coordinate transitions of care, and improves the efficiency of the operation of the medical home.  The Care Coordinator looks after the patient to help optimize his or her health by referring patients to outside community resources and ensuring that patients are aware of their preventive health screenings.   The Care Coordinator’s role is to facilitate medically necessary treatment; collaborate with the patient, physician, and family; and make referrals as needed.  Analyzes data and reports on the patient outcomes to the health care team.

POSITION QUALIFICATIONS:

EDUCATION:           Graduate of an accredited school of nursing; BSN required.

LICENSURE/REGISTRY/CERTIFICATION:        Valid State of Michigan registered nursing licensure; Possesses valid driver’s license and proof of automobile insurance; CPR certification.

EXPERIENCE:            Two years’ experience as a RN in an acute care setting, preferably in case management.  Management/Supervisory experience preferred.

  • Demonstrates ability to analyze and interpret large volumes of data
  • Excellent communication skills, both written and verbal
  • Excellent computer skills including Microsoft Office
  • Excellent organizational skills
  • Ability to handle routine work, unexpected priorities, and multi-task
